Fraud and Abuse Waivers

  • ACO Pre-Participation Waiver:
    The following information describes each arrangement for which our ACO seeks
    protection under the ACO Pre-Participation Waiver, including any material amendment or modification to a disclosed arrangement: N/A
  • ACO Participation Waiver:
    1. Effective November 15, 2023, CNCN entered into an Infrastructure Support Program Agreement (the "Agreement") with PrimaCare, P.C. to support the employment of Nurse Quality Coordinators by PrimaCare, P.C. The Nurse Quality Coordinators will identify and manage actionable clinical interventions among the Medicare population to improve quality of care and patient outcomes
    2. CNCN has developed a Participation Waiver Program Agreement, pursuant to which CNCN will contract with a select group of skilled nursing facilities ("SNFs"), which are participating in the CNCN MSSP ACO, to incentivize high quality and cost-effective performance through health care reform, enhance the coordination of care across the continuum, and improve the quality and efficiency of care experience by CNCN ACO beneficiaries. The agreement will create a network that rewards high performing SNFs based on quality and efficiency metrics and ensures high-value SNF partners are an option for CNCN MSSP ACO physicians and beneficiaries.
